Our team will see your workforce and families for preventative and wellness visits, acute care “sick” visits, follow ups, medication evaluations, women’s health, sports physicals, well child visits, presurgical clearances — all for zero cost to the patient. The goal is to reduce Urgent Care visits, ER visits, and unnecessary referrals and to help find and prevent major medical conditions from developing.
Each situation is unique, but in general, clients choose high deductible health insurance plans or “sharing” or “co-op” plans. Customized Health Care Services works with knowledgeable, experienced insurance brokers who can advise a health insurance plan that pairs well with Direct Primary Care. We can facilitate those connections for you.
For individuals and families desiring even more personalized care or with special needs, Customized Health Care Services offers a concierge plan. Customized Health Care Services provides basic women’s health services at no extra cost. Additionally, we refer patients to obstetrics and gynecology providers for more involved women’s health services.

CHCS negotiates extremely attractive, direct-market rates for all of its ancillary services. Rates are at 60-80% discounts from the big Health System prices. These enormous savings are passed on to the patients and/or their employer. How these charges are handled depends on what type of EHP is in place. With large businesses on self-insured plans, these services are typically passed onto the employer, creating cost savings with every encounter.
With smaller businesses, we usually ask for the employer to cover the nominal costs of wellness labs and typical Point of Care testing. This incentivizes our service for the employee and promotes greater health and wellness via no-cost barriers to care.
Direct Primary Care refers to our basic function at CHCS of providing high-quality, accessible primary care by board-certified physicians to both families and workforces. However, CHCS continues to expand and develop its relationships with other independent clinics, services, specialists and even surgeries. We also coordinate our services with your benefit plan administrators and brokers to identify where we need to focus our efforts and mitigate large medical claims before they ever happen.
Thus, CHCS is acting as a shepherd for the patient, as well as for the organization. It is this education and medical advice that we call “Physician Guided Care.”
Customized Health Care Services will provide you and your employees with direct access to personalized, high quality primary care by physicians, either in whole or in part with existing network physicians. Your contract enables your workforce to see board-certified physicians at a zero-cost option for them. We do this with a standard PMPM model, priced according to the demographics of your firm. We can completely replace any programs you currently pay for.
Our team not only provides excellent preventative care and wellness advice, but also is available for acute care needs as well. Our physicians and APPs have spent years working in Urgent Care clinics, hospitals, Intensive Care Units and Emergency Rooms. We help businesses avoid highly expensive, unnecessary ER visits, referrals and ancillary services.
Your employees will experience zero-cost, scheduled wellness exams and check-ups. They gain unprecedented access to board-certified physicians, the doctors you used to have to wait weeks or months to see. Employees can easily schedule appointments with us.
In particular, we focus attention on employees with medical issues that result in missed workdays. Additionally, we focus attention on employees that are at high risk of serious medical illness. At no cost to them, your employees can utilize our expertise to dramatically decrease their out-of-pocket health care costs, copays and deductibles.
Because of our operational design and staffing, we accommodate most acute-care requests within 1 business day, if necessary. We can perform many of the same functions as an Urgent Care or low-level Emergency Room visit by a physician.
Yes, if you have registered on our portal and have the necessary applications, we can provide televisits, though most patients prefer to be seen in person.
